In order to purchase a Lifetime License, your dog MUST be micro-chipped or tattooed. You AND your VETERINARIAN must complete and SIGN the Permanent Identification Verification Form, which will be uploaded while checking out.

To buy a Senior / Disabled License, you must be 1) at least 65 years of age or 2) a "person with a disability" as defined in Section 102 of the Pennsylvania Dog Law (PL 784, No. 225 as amended) and acknowledge compliance with this affidavit later while checking out.
