To purchase a Lifetime License, your dog must be micro-chipped or tattooed. You also need to complete and attach the Permanent Identification Verification Form later while checking out.
To buy a Senior / Disabled License, you must be 1) at least 65 years of age or 2) a "person with a disability" as defined in Section 102 of the Pennsylvania Dog Law (PL 784, No. 225 as amended) and acknowledge compliance with this affidavit later while checking out.
